Read:
When you visit an office or use a service in your community, you may need to fill out a form to give information about yourself. This information will help the staff (the people who work in the office) to know more about you and to help you better.
This is some of the information you might write on a form:
Gender (or Sex):
Men and boys can write “M” or “male”. Women and girls can write “F” or “female”.
Birthdate / Date of Birth (or DOB)
The date is usually written with numbers, in this order: month / day / year
You may see mm / dd / yy, which means two (2) digits for the month (mm), the day (dd), and the year (yy).
March 12, 1982 is 03 / 12 / 82
October 4, 1965 is 10 / 04 / 65
If you see mm / dd / yyyy, this means you should write four (4) digits for the year (yyyy).
December 17, 1974 is 12 / 17 / 1974
Language:
You can write your native language and any other languages that you speak.
Social Security Number (or SSN):
This is an identification number from the US government that you need to get a job and get other government services. A Social Security number is a 9-digit number, like these:
987-65-4321
469-23-1578
If you don’t have a SSN, you can write “N/A”, or “not applicable”.
Your SSN is important personal information. You need to be careful and safe when you share it. You should only give this number to official organizations and agencies, such as your bank, your doctor's office, your insurance company, your school, or a government office.
You might also write information such as your:
- Name
- Address
- Telephone Number (home phone and/or cell phone)
- Email Address
